This form is to be completed when 15% or more of your centre or kōhanga (including staff and tamariki) have a suspected outbreak. If you have any questions please email HealthProtection@huttvalleydhb.org.nz.
Please note not to put any personal health information into this notification. Other than the name and contact number of the person notifying.
An outbreak is:
- Three or more cases (staff or tamariki) in one area (e.g. under-twos room) showing similar symptoms of illness in 48 hours OR 15% of the centre are unwell.
