This form is to be completed when you suspect you have an outbreak, which is defined as 15% or more of your school (including teachers and students) are unwell.
Include the name and contact number of the person notifying the outbreak, but do not put any personal health information into this notification.
An outbreak is:
- When 15% of the school is unwell
